You’ll work closely with doctors, nurses, and other professionals to undertake a range of activities to facilitate discharges within the Discharge to Assess Framework, including assessing for and arranging nursing and residential placements and generic packages of care.

Hospital discharge social workers help service users with the following:

  • Hospital discharge to their home or another place of care, such as nursing and residential placements
  • Getting in touch with agencies who can help them, especially after discharge
  • Helping them to keep safe
  • Offering advice about money and benefits
  • Helping them make important social and medical decisions
  • Hospital travel costs and applications to charities
  • Helping with any housing worries
  • Offering guidance and support to family and primary carers


We’re looking for qualified social workers with a degree in social work (or equivalent) who are registered with Social Work England, eligible to work in the UK and hold post-qualifying experience in:

  • Adult social care
  • Completing Mental Capacity Assessments/ Best Interest Meeting
  • Undertaking person-centred assessments of adults and their families and carers
  • Setting up packages of care and support planning
  • Dealing with safeguarding enquiries and referrals

It’s also important that you’re up to speed with the relevant legislation.


You’ll be supporting adult service users requiring assessment and delivering services to support their safe and timely discharge from hospital. As a hospital discharge social worker, you’ll also be working with service users with cognitive impairments.
